College Scholarship

This application is to filled out and turned in by the Annual State Meeting. Due by August 15 each year.

This form is to apply for funds supplied by the Kansas T&T Association to the clubs who supply judges at the Kansas meets. 

This form is required for any individual or club to receive funds from USAG. You MUST submit this form every time after applying for funds through the state, wether it be judges pay or the Club Judges Bonus. Fill it out and email it to hyoung@usagym.org OR fax it to 317-237-5069 Attention to Holly Murray. They will hold your check until they get your W-9.

Coaches must fill out the following form in order recieve approval PRIOR to competition in Levels 8, 9 and 10.
